Hope and Hearts 4 Paws Adoption Application

Complete this application and upon approval to adopt a Hope & Hearts 4 Paws furbaby a volunteer will contact you for an interview. This information will help HH4P to achieve its goal of finding permanent, responsible, secure, and loving homes for our furbabies.

Adoption fees begin at $300.00. Personal checks are not accepted without prior approval. Since HH4P is a 501c3 organization, your adoption fee will assist in saving another life. Our organization depends solely on donations to continue to provide the LOVE and care for our rescued furbabies. All dogs are fully vetted, spayed/neutered, heartworm negative/treated.

In order to be considered as an adopter you must:

 

               Be at least 21 years of age.

​

               Have the knowledge and consent of your landlord, if renting.

​

               Be able and willing to spend time and money necessary to provide training, medical treatment

               and proper care for a pet.
